To enroll, please download and complete all of the forms below.  
Please submit all of the completed forms in addition to the following:

• - Birth certificate
• - Immunization Record
• - Registration Fee
• - Curriculum/Book Fee
• - Tuition Fee

• This BCS Student Enrollment Application is specific to US based students.   
• It includes Emergency Contact information, Dismissal Authorization, Field Trip release and Family Agreement forms.  
• Please sign each signature block.

• Please provide the completed State of Illinois Certificate of Child Health Examination
• or Certificates of Religious Exemption to Immunizations
• or Physician Medical Statements of Medical Contraindication.

• Proof of dental exams must be provided for all children in Kindergarten, 2nd, 6th, and  9th grades.  
• All exams must have taken place within 18 months prior to May 15 of the school year. 

• Proof of an eye examination by an optometrist conducted within one year prior to the first day of the school year.

• All students must submit a signed insurance waiver indicating they have medical insurance coverage.

• Please provide a certified copy of student transcripts or complete this document and return with your registration forms as soon as possible.
